Based on record review, the licensee did not comply with the section cited above LPA observed infant (C-1) did not have a Individual Infant Sleeping Plan on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit a completed and signed Individual Infant Sleeping Plan for infant (C-1) to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above LPA observed the stairs barricade was not place in the stairs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit photo of barricaded stairs to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above LPA observed Clorox wipes in day care children bathroom cabinet, and detergent in laundry room that was unlocked which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit photo of locked toxic and picture of locked laundry room door to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above LPA observed two play yards had blackout cover which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit a written statement of her understanding of 102425(b) regulation. Licensee shall remove all play yard blackout covers and submit photo to CCL by POC due date.

Based on record review, the licensee did not comply with the section cited above LPA observed Assistant (S-1) did not have immunization record for pertussis and measles which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit Assistant (S-1) immunization records for pertussis and measles to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above LPA inspected the backyard and observed dog feces, gardening tools, dollys, ladders, sharp and hazardous items which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit a written statement of her understanding of 102417(b) regulation. Licensee shall remove all dog feces, clean backyard, and remove all sharpe and hazardous items. Licensee shall submit photos to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above fireplace in living room was not screen or barricaded which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit photo of screen or barricaded fireplace to CCL by POC due date.

Based on records reviewed, the Assistants did not have proof of immunizations on file. That poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit proof of Assistants Immunization records.

Based on records reviewed, one Assistant ( S-1) is not associated to the facility. That poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit documents to CCL to get Assistant (S-1) associated to the facility.

Based on records reviewed, Licensee and Assistants did not have proof of influenza or statement of opting out in file. That poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit documents for influenza to CCL for her and Assistants.

Based on records reviewed), the Licensee has expired Mandated Reporter Training & Assistants did not have proof of completion on file. That poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that herself and Assistants will complete the training and will submit proof of completion.
