Based on observation, the licensee did not comply with the section cited above in which play yards did not have fitted sheets which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will purchase and put fitted sheets on the play yard mattresses and will submit proof of placement of the fitted sheets to the San Jose Regional Office (SJRO) by close of business on June 8, 2022.
Based on record review, the licensee did not comply with the section cited above in which Child 03 is missing their immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will request immunizations from the program they participate in and will submit a copy of a completed PM 286 for Child 03 to SJRO by close of business on June 8, 2022.
Based on record review, the licensee did not comply with the section cited above in which Child 07 was missing LIC 9227 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide Child 07's parents LIC 9227 to be completed, licensee will submit a copy of the completed form to the SJRO by close of business on June 8, 2022
