Barnes met with Site Supervisor Sheila Acosta, explained the purpose of the visit, and was granted access. Acosta was provided with the LIC125 Entrance Checklist. Present during the inspection were Acosta, eight teachers, and forty-one preschoolers. The facility operated Monday through Friday from 7:30am to 5:30pm and had four preschool classrooms. Parents provided meals, though meals could also be purchased. All meals brought from home were labeled with the child’s name and refrigerated upon arrival. Lunch boxes included ice packs. Scheduled mealtimes included breakfast, morning snack, lunch, and an afternoon snack. Barnes observed the kitchen to be clean and organized. Meals prepared by the facility were made using the microwave, refrigerator, stove, and oven. The sink had hot and cold running water, which the Site Supervisor stated was used only for washing. Filtered water from dispensers was used for cooking and was available throughout the day in each classroom. Sink water was not used for drinking Each classroom was equipped with a built-in fire and smoke alarm with carbon monoxide detection, and the hallway contained the same combination system. All required postings were observed to be publicly visible. The most recent fire drill had occurred on October 13, 2025. LPA Barnes reminded the Site Supervisor of the requirement to conduct and document fire drills at least once every six months. Five 3A-40-BC fire extinguishers, last serviced on September 24, 2025, were observed on site. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies: Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care. Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care. PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency. LPA met with the Owner/ Licensee Evelyn Weimer, and explained the nature of today's inspection to her. There were a total of 42 preschool age children at the facility today along with 10 fully qualified teachers, and 7 assistants. This case management inspection was made to inquire about an unusual incident that occurred on Monday April 17, 2023. During today's visit LPA Macias toured the facility, interviewed staff, reviewed the LIC 624, and obtained copies of pertinent information. Based on staff interview, as well as the self reported incident report; a child found medication on the classroom floor and picked up the medication. The medication belonged to an employee and was disposed of. Prior to today's inspection the employee involved received disciplinary action, the center's staff medication policy was updated, and a staff training is scheduled for today April 18, 2023. LPA Macias reviewed the importance of medications, disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. Storage areas for poisons/ medications shall be locked. As a result of this inspection, a deficiency was cited. Appeal right were printed and reviewed with the Owner/ Licensee. LPA Macias reviewed the LIC9224, acknowledgement of receipt of licensing report. Type A deficiency cited, exit interview conducted, and a copy of this report was reviewed by the Owner/ Licensee Evelyn Weimer.
