Resolution: Licensee shall submit to licensing Program before 1/05/24 a copy of the log documenting the infant sleep for the period 12/21/23 to 1/4/24. (log provided to licensee) Licensee will continue documenting the infants sleep until they reach the age of two years old.

Resolution: Licensee shall renew the Mandated Reporter training and will submit a copy of the certificate to Licensing Program before or on 1/05/24

Resolution: Licensees Guadalupe and Monica shall renew the Mandated Reporter training. Licensees will submit a copy of the certificate of completion to the Licensing Department not later than on 12/17/21.
