Based on observation, the licensee did not comply with the section cited above fire extinguisher was last service on 09/10/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will service fire extinguisher and submit proof to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above during outdoor inspection LPA observed a gallon of paint in top of a shelf where day care children play which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Deficiency cleared during today's inspection. Licensee removed then item during visit.

Based on record review, the licensee did not comply with the section cited above LPA did not observe a written statment stating Licensee is opting out of receiving the flu shot which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit a written statement stating opting out of receiving the flu shot to CCL by POC due date.

Based on record review, the licensee did not comply with the section cited above in child C-7 did not have a LIC 9227 in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall complete the LIC 9227 form for child C-7 and submit a copy to CCL by POC due date.

Based on LPA observation, the licensee did not comply with the section cited above in bouncer was in the living room, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed bouncer during visit. Deficiency cleared today.

Based on LPA observation, the licensee did not comply with the section cited above no Notification of Parents' Rights Poster was observe posted in a prominent, publicly accessible area at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee posted the Notification of Parents’ Rights Poster in a prominent, publicly accessible area near entrance. Defieciency cleared today.

Based on record review, the Licensee did not comply with the section cited above in which child C3 is missing LIC 700, and children (C1, C2, and C4) are missing LIC 627 in their file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LIC 700 to child C3 parents and children (C1, C2, and C4) parents with LIC 627 and will submit completed forms to San Jose Regional Office (SJRO) by POC date 08/09/2022.

Based on LPA observation, the licensee did not comply with the section cited above in which child C1 is missing LIC 9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LIC 9227 to child C1 parents and will submit completed form to San Jose Regional Office (SJRO) by POC date 08/09/2022.
