Based on record review, the licensee did not comply with the section cited above, 3 of 3 files had expired mandated reporter trainings. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistances will complete Mandated Reporter Training and submit proof via text message/picture by POC day 7/21/2025.

Based on observation, the licensee did not comply with the section cited above. Licensee is licenseed for 8 and had 9 child in care, 3 of which are infants. It should be noted that Licensee has been over capacity multiple times, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LIcensee agrees to watch the childcare video "How many Children Can Attend A Family Child Care Home", available at www.ccld.ca.gov under the section resources for providers. Provider agreed to submit a written memo, outlining her understanding of requirements, and importance for abiding by licensing capcity/ratio requirements. Memo to be submitted by POC date.

Based on record review, the licensee did not comply with the section cited above. Licesee failed to have Individual Sleeping Plans for the infants in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall have parents complete and sign LIC 9227 Individual Infant Sleeping Plan. Licensee shall watch the Safe Sleep Video available at www.ccld.ca.gov under the section of Safe Sleep. Provider agreed to submit a written memo, outlining her understanding of requirements. Memo to be submitted by POC date.

Based on record review, the licensee did not comply with the section cited above in 4 out of 4 infants did not have documentation the the Licensee was checking and documenting while the infant was sleeping which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send proof of a month of documentation to LPA of checking infants while they are sleeping.

Based on record review, the licensee did not comply with the section cited above in 3 out of 5 children present did not have immunizations in their file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send proof of children's immunizations to the LPA by the above POC date.

Based on observation the licensee did not comply with the section cited above in when LPAs arrived to the home there were 9 children in care and Licensee only has a License for 8 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee had two children picked up when LPAs were present; LPAs communicated with Licensee that she is Licensed for 8 children and can not exceed her capacity.
