Resolution: The Licensee was provided a blank fire drill log. She was advised to conduct and document a drill at least once every six months. Licensee can submit proof of correction by POC date of 10/27/2023.

Resolution: The Licensee was provided Infant Safe Sleep regulations and explained that even though the sleeping infants are observed, Licensees are still required to document naps for children under 2 years old.

Resolution: Licensee and her daughter will ensure to take the required training online at www.mandatedreporterca.com both certificates can be e-mailed to LPA.

Resolution: Licensee and her assistant will ensure to renew the CPR and First Aid by POC date of 10/27/2023. Proof of certification can be sent to LPA's email.

Resolution: Licensee was provided an updated PUB394 poster. She will ensure to post it in a prominent area of the childcare. Licensee can send a photo of the poster.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs reviewed five children's records, two files were missing and the other three were incomplete.

Resolution: Licensee was provided blank forms and LIC311D. Licensee also requested to be referred to TSP program for assistance with record keeping.
