LPAs observed pests in facility during inspections on 28 August 24 and 18 September 24 which poses a potential health, safety, or personal rights risk to persons in care.
Based on observation, interview, and record review, the facility did not comply with the section cited above by having an incident where Staff #1 handled a child in a rough manner during naptime which poses an immediate health, safety or personal rights risk to persons in care.
