Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.

This requirement is not met as evidenced: LPAs learned that Staff #1 places children into a high chair for a time out when children misbahave, hit or bite other children. This is an immidiate risk to children in care.

Based on interviews and record review the facility did not comply with the above regulation as there was not direct supervision of the children which poses Health, Safety, or Personal Rights risk to persons in care
