Based on interview, the licensee did not comply with the section cited above in which Licensee did not record 15 minutes logs and document infant's: labored breathing, signs of distress - which includes but is not limited to flushed skin color, increase in body temperature and restlessness. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email LPA Vang at padao.vang@dss.ca.gov stating that she has reviewed and understands all safe sleep regulations. Licensee will also email LPA Vang proof of 2 pages of 15 minutes logs.
Based on interview, the licensee did not comply with the section cited above in that Licensee was not aware that she is to document infant's: labored breathing, signs of distress - which includes but is not limited to flushed skin color, increase in body temperature and restlessness. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with safe sleep regulations. Licensee will email LPA Tillery stating that she has reviewed and understands all safe sleep regulations.
Based on interview and record review, the licensee did not comply with the section cited above in that Licensee does not have a current Mandated Reporter Training Certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit Mandated Reporter Training certificate to LPA Tillery by plan of correction date.
