Based on record review, the licensee did not comply with the section cited above, Licensee and Assistants have expired mandated reporter certificates, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she shall take the mandated reporter training. Assistants shall take mandated reporter training also. Licensee shall send certificates of completion to LPA by email. LPA provided contact information.
Based on observation and record review, the licensee did not comply with the section cited above due to not having at least one child is enrolled in and attending kindergarten or elementary school and a second child is at least six years of age. Which poses an immediate health, and safety risk to persons in care.
Resolution: The Licensee stated that she will ensure she remains in ration starting tomorrow. LPA Barragan will return to the facility within the next 10-days to ensure the facility remains in ratio and to clear the plan of correction.
