Based on observation, interview, record review, the licensee did not comply with the section cited above in that Staff #4 and Staff #6 were present and working prior to being fingerprint cleared, which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above as the wood chip cushioning was low and the dirt are exposed in several areas, which posed a potential health, safety or personal rights risk to persons in care.

(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met, evidenced by: Based on observations and staff interviews, there is a lack of supervision during transistion periods, and staff are unable to provide 100% supervision.

(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.

(e) As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls.
