Based on observation and interview, the licensee did not comply with the section cited above by not having a full size 2A-10-BC fire extinguisher at facility which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a full size 2A-10-BC fire extinguisher for facility and email LPA evidence of its presence in facility by 18 December 2024.

Based on observation and interview, the licensee did not comply with the section cited above by having cleaning compounds accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will properly store cleaning compounds in facility and email LPA evidence of their safe storage in facility by 18 December 2024.

Based on observation and interview, the licensee did not comply with the section cited above by having poison bug killer in unlatched, unlocked kitchen cabinet under sink which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a lock for their storage closet to store chemicals in to bring facility back into regulation and email LPA evidence of its presence in facility by 18 December 2024.

Based on observation and interview, the licensee did not comply with the section cited above by not having both sides of stairs in facility fenced or barricaded when children under 5 are in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will get a gate or barricade for both sides of stairs and email LPA picture of its existence in facility by 17 January 2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by not conducting and documenting fire drills on a fire drill log at least once every 6 months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill at facility, document its occurrence on a fire drill log, and email LPA picture of filled out log by 17 January 2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by not having a current mandated reporter certificate for licensee or assistant which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant will complete mandated reporter training and will email LPA completed certificates by 17 January 2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by not having a facility roster for children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have a completed Roster for all children in care and will email completed roster to LPA by 17 January 2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by not maintaining sleep logs for infants in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will start filling out 15 minute check sleep logs for all napping infants in care. Licensee will email LPA 2 weeks of completed sleep logs by 17 January 2025.
