Based on observation and interview, the licensee did not comply with the section cited above. LPA observed a crib that has been recalled, mattress not cover with fitted mattress, jungle gym hanging above the crip, toys and lose blank inside the crib which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, will remove crib and purchase a porta-crib by due date. Will submit photos of correction.
