an Unusual Incident Report received on 01/20/202. Facility self-reported that on 01/16/2026 Child 1 and Child 2 eloped from the facility unsupervised. The children were found at the residence of Child 1 approximately 30 minutes after the facility was aware the children were not present. This posed an immediate risk to the health and safety of children in care.
(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents.
California Lead Action Level at Child Care Centers. b) Testing results with fractional ppb readings of 0.5 or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. 1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidenced by: results from a lead test of a faucet/fountain on 12/09/22 that indicated an exceedance of 5.5 ppb. which posed a potential health, safety or personal rights risk to persons in care.
