Based on observation, the licensee did not comply with the section cited above in LPA observed the tire thread to be showing and obserevd the carpet on the ramps to be peeling off which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in conducting drinking water tested for lead contimination before January 1, 2023, which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in 3 of 3 staff did not have current Mandated Reporter Child Care Provider (AB1207) training completed, which poses a potential health, safety or personal rights risk to persons in care.

(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.

Based on record review, the licensee did not comply with the directive above, as Sink Faucet- H (6.6 UG/L) tested with an Action Level Exceedance (ALE). This poses an immediate Health and Safety risk to children in care.
