Based on observation, interview, record review, the licensee did not comply with the section cited above in having safe sleep logs on three infants which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will complete safe sleep slogs and provide proof of completion to LPA.
Based on observation, interview, record review, the licensee did not comply with the section cited above in not providing a record of the fire/disaster drill which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will complete disaster drill with children and send proof of completion to LPA.

Resolution: Licensee stated that she has assitant immunizations but misplaced the file. Licensee states she will send proof of certification completion to LPA.
Based on observation, interview, record review, the licensee did not comply with the section cited above in not providing proof of CPR/First aid completion for licensee and assistant which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that licensee and assistant have completed CPR/FA however licensee misplaced the file. Licensee will provide proof of completion to LPA.
Based on observation, interview, record review, the licensee did not comply with the section cited above in by not having assistant file available for review during inspection (LIC 9051, LIC 9108, TB results) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that she had assistant file but misplaced it. Licensee states she will provide proof of completion to LPA.
Based on observation, the licensee did not comply with the section cited above. Licensee could not find her carbon monoxide detector, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send a picture of the carbon monoxide in operable condition, and send proof of completion to LPA Tamayo no later than 03/28/24.

When LPA arrived to the facility, there were 12 children (3 infants and other ages children) with an assistant until licensee's daughter arrived to the home, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee's daughter arrived to the facility. The licensee was arrived aroung 11:30.

Resolution: The licensee will schedule CPR/First Aid training and after the licensee will submit the certifications to LPA

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. LPA observed a object (mobile) haning in the playpens. A infant was sleeping on the bouncy seat with a blankety cover on the top, A blanket under the infant when the baby was laying down,which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA talked to the licensee and her daughter. They put the infant in the playpen, removed the blanket and mobile.
