Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current mandated reporter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete mandated reporter (AB1207) training and submit a copy of their certificate to CCL by 2/28/25.
Based on observation, the licensee did not comply with the section cited above in that a baby walker was observed in the daycare area, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Immediately, Licensee removed walker during inspection. Licensee agrees to submit a written statement acknowledging they understand baby walkers are prohibited. Licensee will submit correction to CCL by 1/31/25.
Based on observation, the licensee did not comply with the section cited above in that due to the age of the children present facility was not in ratio, which poses an immediate health, safety or personal rights risk to persons in care. During inspection there were 3 infants and 4 children from the ages of 2-4 years old.
Resolution: During inspection C6 was picked up bring facility back into ratio. LPA provided Licensee a copy of capacity regulations poster and went over the age requirement's needed to care for the maximum number of children. Licensee agrees to submit a detailed plan on how they will maintain ratios. Licensee will submit correction to CCL by 1/31/25.
Based on observation, the licensee did not comply with the section cited above in that several items which pose a danger were accessible, which poses an immediate health, safety or personal rights risk to persons in care. The following accessible items were found during inspection - Daycare Bathroom: under the sink was a bottle of baby powder and soap which contained keep out of reach warnings. Kitchen: under the kitchen sink was a bottle of clorox spray and soap which contained keep out of reach warnings. Kitchen: several knives and scissors were accessible in a lower drawer.
Resolution: Immediately, Licensee removed the items during inspection. Licensee agrees to submit a detailed plan on how they will keep dangerous items inaccessible. Licensee will submit correction to CCL by 1/31/25.
Based on record review and Licensees admission, the licensee did not comply with the section cited above in 3 out of 3 infants in care did not have sleep logs, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Immediately, Licensee agrees to conduct and document 15 minute sleep checks. LPA provided Licensee a copy of the Infant Safe Sleep regulations in Spanish and requested Licensee submit a written statement acknowledging they have read and will follow the regulations. Licensee will submit correction to CCL by 1/31/25. Additionally, Licensee agrees to submit completed sleep logs.
Based on observation and Licensee admission, the licensee did not comply with the section cited above in 1 out of 7 children present were sleeping in an off limit room, which poses an immediate health, safety or personal rights risk to persons in care. C1 was sleeping in a bed room which is listed as off limits.
Resolution: Immediately, Licensee removed C1 from the off limits room. Licensee stated they would like to keep the room off limits and will no longer use the room. Licensee agrees to submit a written statement saying they will not use off limit areas for day care use. Licensee will submit correction to CCL by 1/31/25.
Based on observation and Licensee admission, the licensee did not comply with the section cited above in 1 out of 3 infants was not sleeping in a crib, which poses an immediate health, safety or personal rights risk to persons in care. C1 was sleeping on a bed in an off limit room.
Resolution: Immediately, Licensee removed C1 from the bed and placed them to sleep in a crib. LPA provided Licensee a copy of the Infant Safe Sleep regulations in Spanish and requested Licensee submit a written statement acknowledging they have read and will follow the regulations. Licensee will submit correction to CCL by 1/31/25.
Based on observation, the licensee did not comply with the section cited above in 3 out of 3 infants had loose articles in their cribs, which poses an immediate health, safety or personal rights risk to persons in care. C4 and C6 had blankets covering them and C1 had a loose blanket under them.
Resolution: Immediately, Licensee removed the blankets during inspection. LPA provided Licensee a copy of the Infant Safe Sleep regulations in Spanish and requested Licensee submit a written statement acknowledging they have read and will follow the regulations. Licensee will submit correction to CCL by 1/31/25.
Based on record review, the licensee did not comply with the section cited above in the licensee failed to renew their training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866. which poses/posed a potential health, safety or personal rights risk to persons in care, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agrees to submit proof of registration to renew their training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866.in through an EMSA approved vendor on or before 12/01/2023 and submit proof of the training certification upon completion, to the department by fax, mail or e-mail.
Based on observation, the licensee did not comply with the section cited above in LPAs observed several kitchen knives, lighters with lighter fluid, kitchen scissors in a lower kitchen cabinet, 1 set of kitchen knives in a block are located in kitchen accessible to older children in care, aerosol air fresheners underneath the restroom sink; shampoos, hygiene products in the shower, the hall closet, 1 air freshener spray can 5 bottles of essential oils and in the living room TV stand; 1 can of air fresher, 1 Clorox and 2 generic all purpose cleaning spay bottle and 1 spray can of glass cleaner under the kitchen sink, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee agrees to ensure the home shall be free from defects or conditions which might endanger a child and to include, poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children and will place a lock on cabinets to make poisons, toxins, cleaning supplies, medicines, household products and other dangerous knives and/or weapons inaccessible to children. The licensee also agrees to submit proof of locks or latches placed on storage cabinets to make inaccessible to children and submit the proof by photo, to the department on or before 11/03/2023.
Based on record review, the licensee did not comply with the section cited above in the licensee failed to conduct a fire drill with daycare chidlren after 02/2023, as required which poses potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agrees to conduct fire drills and disaster drills at least once every six months beginning the week of 11/06/2023 and submit proof of the fire drill to the department on or before 11/08/2023.
Based observation, the licensee did not comply with the section cited above in at 10:47 AM LPAs observed a subject child sleeping in a play yard, with a milk bottle propped in their mouth by a blanket, a stuffed animal pillow and a large folded mattress pad laid on top of the mattress, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agrees to ensure cribs or play yards shall be free from all loose articles and objects for all infant chidlren enrolled and cared for at the facility.
Based on record review, the licensee did not comply with the section cited above in the licensee failed to ensure documention of a subject child's immunizations as required by the California Code of Regulations, Title 17, Section 6070, and maintain such documentation for a subject child enrolled and present during the inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agrees to document each child's immunizations as required by the California Code of Regulations, Title 17, Section 6070, and shall maintain such documentation for as long as the child is enrolled and submit proof of immunizations records for a subject child on or before 11/08/2023 by fax, mail or e-mail.

Resolution: The licensee agrees to documentation and maintain the time of each 15-minute infant safe sleep checks in the each infant’s file, ensuring the documentation is available to the department for review and submit proof of documentation for a subject infant child enrolled and attending on or before 11/08/2023 by fax, mail or e-mail.
