Based on observation, interview and record review, the licensee did not comply with the section cited above in the LPA observed 2 daycare children in strollers with blankets, and one 1 of the 2 children asleep which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to ensure all children receive safe, healthful, and comfortable accommodations, furnishings, and equipment, which includes equipment designated for napping and refrains from using other equipmmen not designated for napping at family child care home. The licensee also agrees to submit a written statement of understanding the Title 22 Personal Rights regulation and infant safe sleep regulations, to the department by mail.

Based on observation and interview, the licensee did not comply with the section cited above in the LPA observed a inflated swimming pool with 9 feet long, 6 feet wide, with 6.5 inches of sitting water, and a small ladder placed ouside of the pool to provide access, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to ensure compliance with the above Title 22 regulations 102417(g)(5)(A) and 102417(g)(5)(B), as well as all regulations related to large bodies of water, to make bodies of water inaccessible to children. The licensee also agrees to submit a written statement of understanding the regulations for large bodies of water to the department by mail. S.Brewer

Resolution: Licensee conducted fire drill during this visit.

Based on (record review) and interview the licensee did not comply with the section cited above in that C3 did not have 15 minute safe sleep tracking which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee implemented 15 minute log during this visit- LPA provided 15 minute safe sleep log.

Based on record review, the licensee did not comply with the section cited above licensee did not have LIC 9052 for licensee's spouse/ assistant for review during LPA's inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provided copy of LIC 9052 to Community Care Licensing by 01/20/2022.

Based on record review, the licensee did not comply with the section cited above in C3 Immunization records were not available for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provided copy of immunization records to Community Care Licensing by 02/13/2022.

Based on records review, the licensee did not comply with the section cited above in licensee's assistant immunization records missing TDAP and Flu or statement declining Flu vaccine which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to provide proof of immunization for assistant to Community Care Licensing by 02/13/2021

Based on record review, the licensee did not comply with the section cited above. Licensee and licensee's assistant 's Mandated Reported Certificates were not available for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to provide Mandated Reported Certificates by 01/27/2022.
