Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current mandated reporter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to completed mandated reporter child care provider training (AB1207) and submit a copy of the certificate to CCL by 2/9/26.
Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current CPR/First Aid certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to completed CPR/First Aid training (American Heart Association, American Red Cross, EMSA and submit a copy of certificate to CCL by 2/9/26.
Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure their mandated reporter training was maintained at the facility which poses a potential safety risk to children in care.
Resolution: LPA discussed the requirement to maintain personnel records at the facility. LPA also discussed the mandated reporter training requirement and reminded Licensee to renew the course every two years. Licensee agreed to provide completed certificate to LPA by 02/27/2025.
Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure their EMSA pediatric CPR and first aid certification was maintained at the facility which poses a potential safety risk to children in care.
Resolution: LPA discussed the requirement to maintain personnel records at the facility. LPA also discussed the requirement to renew CPR and first aid certification every two years prior to expiration. Licensee agreed to provide proof of enrollment and/or completed certificate to LPA by 02/27/2025.
Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 (L1) Licensee which posess potential health, safety or personal rights risk to persons in care.
Resolution: Licensee plans to complete Mandated reporting certificate and submit Certficate to LPA Steven Montoya (steven.montoya@dss.ca.gov) via email by 9-8-2023.
Based on record review, the licensee did not comply with the section cited above in 1 out of [1] record review, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete First Aid CPR and submit to LPA Steven Montoya (steven.montoya@dss.ca.gov) via email by 9-8-2023.
