Based on Licensee's admission and record review the licensee did not comply with the section cited above in that 15 minute tracking is not being completed for infants enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will implement 15 minute tracking and submit logs for each infant enrolled by POC due date 10/02/23. LPA provided a sample 15 minute tracking form.

Resolution: Licensee will submit completed immunizations for S1 and S2 by POC due date: 10/06/2023
Based on [(observation) the licensee did not comply with the section cited above in that LIcensee did not notify the department of changes to facility and using a renovated garage that has not been inspected which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will cease using the garage until inspected by Fire Authority and construction permits/clearances obtained.
