Based on observation, interview and ecord review, the licensee did not comply with the section cited above in the licensee was unable to provide proof of lead testing on the water outlets used for food preparation and located on site, in the school kitchen, which poses a potential health, safety or personal rights risk to persons in care.

Type B citation - potential risk if not corrected

Based on record review and interview, the licensee did not comply with the section cited above in that 2 out of 4 staff records (Staff 2 [S2] & Staff 4 [S4]) did not have a Health Screening available to review. Additonally, 1 out fo 4 staff members (Staff 3 [S3]) did not have any documents available to review which poses/posed a potential health, safety or personal rights risk to persons in care.

.3 (b)(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidence by: Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlet B, with a level exceedance of 6.3 ppb, for a drinking fountain in classroom 1. This is a potential health and safety risk to persons in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents.
