Based on (record review)], the licensee did not comply with the section cited above in C4 is missing personal rights and health history which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on (record review)], the licensee did not comply with the section cited above in that C4 did not have an admission agreement for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on (record review)], the licensee did not comply with the section cited above in that C4 did not have a signed medical consent for review in file which poses/posed a potential health, safety or personal rights risk to persons in care.

.5(b)(1): Teacher-Child Ratio- (1) A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children. This requirement is not met as evidenced by: Based on the agency's self - reported Unusual Incident report on 02/27/2025 and interview- facility staff was left to supervise 18 children alone, from 7:15AM- 7:30AM - This is an immediate health & safety risk to children in care. This is a repeat violation.
