Based on observation, interview, the licensee did not comply with the section cited above in by the directors own admission the staff utilize 1 of the 2 restroom stalls with a toilet and sink, leaving the facility with only 1 toilet and 1 sink designated and/or available for school age children, which poses a potential health, safety or personal rights risk to persons in care.

Based on interview, record review, the licensee did not comply with the section cited above in the licensee failed to ensure a director or staff complete 16 hours of health and safety training if necessary pursuant to Health and Safety Code Section 1596.866., which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in the director was unable to provide proof of documentation of disaster drills conducted which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above in the licensee failed to associate the current Site Director Kimberly Banks, who was present and working at the facility as of 10/31/2023 which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as S1 is missing mealses and pertussis immunizations, which poses/posed a potential health, safety or personal rights risk to persons in care.

In addition the LPA observed kitchen knives located in an open cabinet, which poses an immediate health, safety or personal rights risk to persons in care.
