Based on record review], the licensee did not comply with the section cited above in the licensee did not complete renewal mandated reporter training, when their mandated reporter training certificate expired as of 03/2025, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to ensure a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training, and submit proof of the renewal training certificate by fax, mail or e-mail.

Based on observation, the licensee did not comply with the section cited above in medications/supplements, several kitchen knives, Clorox clean-up wipes, supplement powder mix, air fresheners and dog clean up/freshener and cleaning compounds were accessible in the kitchen drawers/cabinets and on kitchen counter, perfumes, nail polish remover, treatment shampoos and razor shower were accessible in the designated for daycare which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to submit a written statement to ensure the home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not limited to: (4) Poisons, detergents, cleaning compounds, medicines, firearms and other items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. The licensee agrees to submit proof of corrections by photo through fax, mail or e-mail on or before 12/23/2024.

Based on observation and interview,, the licensee did not comply with the section cited above as there are no records of diaster drill and fire drill within the 6 months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licencees agree to complete and document a diaster drill/fire drill and submit proof to the LPA by the POC due date.
