Resolution: The licensee agrees to comply with the regulations by ensuring they report information as required in the Title 22 regulation 102416.2 Reporting Requirements, and submit a written statement of understanding to the department with the updated LIC279 Application form and updated information on their recent name change, by mail.

Resolution: The licensee agrees to ensure the family child care home shall conduct fire drills and disaster drills at least once every six months and document the drills, including the date and time of each drill; This documentation shall kept at the family child care home. The license also agrees to submit proof of documentation of fire drill conducted by fax, mail or e-mail.

Resolution: The licensee agrees to ensure an emergency information card shall be maintained for each child and shall include the child's full name, telephone number and location of a parent or other responsible adult to be contacted in an emergency, the name and telephone number of the child's physician and the parent's authorization for the licensee or registrant to consent to emergency medical care, and submit proof of a subject child's completed records by fax, mail or email.

Resolution: The licensee agrees to adhere to the following requirements by documenting physical checks on the infants in care every 15 minutes; The provider shall check and document the following: Labored breathing, Signs of distress which includes but is not limited to flushed skin color, increase in body temperature and restlessness. In addition the licensee agrees to submit proof of documentation with a statement of understaning the regulation by fax, mail or email. (C) Infants up to 12 month of age who are sleeping in a position other than on their back.

Resolution: The licensee agrees to ensure If no assistant provider is present at a Large Family Child Care Home, then the licensee shall comply with the capacity requirements for a Small Family Child Care Home as specified in subsections (b) and (c). In addition, the licensee agrees to submit a statement of understanding the regulation, proof of schedule to ensure they will not exceed ratio during operating hours, along with proof of a qualified assistant to help with the care of 10 children enrolled.
