CRAMPTON FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in at 9:26 AM during today's inspection 4 of 6 play yards were observed to have loose fitted and dislodged sheets on the mattresses designated for infant children, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to ensure each crib and/or play yard for each infant who is unable to climb out of the crib or play yard, shall have firm mattresses and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged and send proof by photo to the department on or before 12/12/2023 by fax, mail or e-mail.

Based on observation and interview, the licensee did not comply with the section cited above in at 9:26 AM Bedroom 3 designated as a napping room was observed 5 play yards, 1 play yard filled with miscellaneous items, 4 play yards observed to have loose sheets, blankets, pillows and plush stuffed toys. At 9:45 AM LPA S.Brewer observed a 6th play yard located in the designated indoor living room/daycare area to have blankets, pillows and plush toys which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to ensure all cribs and/or play yards shall be free from all loose articles and objects and will provide a written understanding of the Title 22 regulation 102425 Infant Safe Sleep standards, signed by the licensee and staff working with infant children, then submit the signed statement of understanding to the department on or before 12/12/2023 by fax, mail or e-mail.

Based on record review, the licensee did not comply with the section cited above in the licensee failed to renew their training for the Mandated Reporter training after the certification expired on 11/08/2023, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to complete in the mandated reporter training provided and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training, and provide the proof of training certificate to the department on or before 12/29/2023 by fax, mail or e-mail. (LPA provided licensee with a copy of PIN 19-17-CCP, dated November 22, 2019 prior to leaving)

Based on LPA record review the licensee did not comply with the section cited above in 2 out of 2 persons for S1 and S2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit certificates of completion for mandated reporter by POC due date 11/12/2021
