Based on observation, interview, record review, the licensee did not comply with the section cited above by Assistant #2 not having fingerprint clerence in Guardian which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee had employee go get fringerprint today. Her apptointment is at 1:30pm. Licsenee will send proof of completion via copy of live scan form. Once LPA see that fingerprints have cleared in Guardian LPA will notifiy licensee.
