Based on observation and interview, the licensee did not comply with the section cited above, by caring for 10 children without an assistant present, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA Crass a written statement stating she understands the importance of staying within capacity regulations if no assistant present, and that she agrees to comply going forward. She will email this to LPA by 2/13/2024.

Based on observation and interview, the licensee did not comply with the section cited above by not having a working smoke/carbon monoxide detector, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send LPA Crass a video or photo showing that the batteries on the smoke/carbon monoxide detector have been replaced by 2/19/2024.

Based on observation and interview, the licensee did not comply with the section cited above by having 2 out of 2 crib sheets that do not fit tightly, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send LPA Crass a photo showing that the sheets are tightly fitted to the crib mattress, by 2/19/2024.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 5 childrens files, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a completed and parent signed consent for medical treatment form (LIC 627) for the childrens' files who are missing the form, and will email LPA Crass a photo as poof, by 3/11/2024.

Based on observation, interview, and record review, the licensee did not comply with the section cited above by not having a current roster of all enrolled children, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA Crass an up to date, and current roster by 3/11/2024.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 2 out of 5 childrens files, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have the parents sign and completely fill out the bottom portion of the LIC 995A for all children in care, and will email LPA Crass a photo as proof, by 3/11/2024.

Based on observation, interview, record review, the licensee did not comply with the section cited above in conducting fire/disaster drills which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire/disaster drill and send a copy of the information to CCLD.

Resolution: Licensee will submit the cards for C4 and C5.

Resolution: Licensee will provide immunization records for C2, C3 and C5.

Resolution: Licensee will provide records for C4 and C5.

Based on observation, interview, record review, the licensee did not comply with the section cited above in ensuring that all children have the emergency informtion care on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a copies of C4 and C5 to CCLD.

Based on observation, interview, record review, the licensee did not comply with the section cited above in because there is one child who is younger than 12 months that doesn't have an Individual Infant Sleeping Plan which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the Individual Infant Sleeping Plan with parent/guardian and submit to CCLD.
