Resolution: Licensee agrees to obtain immunization record for C3 and a submit a copy to CCL by 2/9/26.
Based on record review, the licensee did not comply with the section cited above in that C3 did not have a 15 minute sleep log on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: License agrees to conduct and document 15 minuet sleep checks on C3 and submit a weeks worth of entries to CCL by 1/16/26. LPA went over the importance of following infant safe sleep regulations.
Based on observation, the licensee did not comply with the section cited above in that the vertical clearance from the ground to the bottom of the fence measured at three inches, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to extend the bottom of their pool fencing ensuring the vertical clearance from the ground to the bottom of the fencing has a clearance of two inches. Once completed Licensee agrees to submit photos to CCL by 3/28/25.
Based on observation, the licensee did not comply with the section cited above in that key lockable device measured at 56 inches, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to move their existing self-closing, self-latching, key lockable device or install a new one at 60 inches above the ground. Licensee agrees to submit photos to CCL by 3/28/25.
