Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current mandated re[porter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit mandated reporter (child care provider) certificate to CCL by 11/27/25.

Based on observation and interview conducted, the licensee did not comply with the section cited above as LPA observed an exersaucer in the home and Licensee disclosed they used the exersaucer for a child (C1) present during water play which poses an immediate safety risk to children in care.

Resolution: LPA discussed with Licensee that baby walkers, bouncers, jumpers, and similar items are not permitted in the facility and cannot be used by day care children. Licensee placed the exersaucer in an off-limits area during the visit.

Based on observation, the licensee did not comply with the section cited above in that cleaning compounds, medicine, tools, hair products and swords were accessible, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During the inspection Licensee removed the cleaning compounds, tools and scissors in the kitchen, medicine and hair products in the bathroom. A bedroom which is off limits was open and LPA observed swords hanging on the wall, Licensee closed the door but was unable to lock it. Licensee agrees to lock the door and submit a written statements detailing how the bedroom will be made off limits and submit to CCL by 2/20/23.

Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a mandated reporter certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
