Based on observation, the licensee did not comply with the section cited above as licensee did not ensure stairs to the second floor were barricaded which poses a potential safety risk to children in care. A civil penalty of $250 is being assessed and provided.
Resolution: LPA discussed the requirement to barricade stairs when there are children less than five years old in care. Licensee placed a baby gate at the bottom of the stairs during the visit.
Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as LPA observed the last fire and disaster drill was conducted on 07/22/2023 which poses a potential safety risk to children in care.
Resolution: Licensee agreed to provide documentation of completed drill to LPA by 07/24/2024
Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure their EMSA pediatric CPR and first aid certification included in-person instruction which poses a potential safety risk to children in care.
Resolution: LPA discussed the requirement to renew CPR and first aid certification every two years prior to expiration. LPA informed Licensee that certifications are to include in-person instruction. Licensee agreed to provide proof of enrollment and/or completed certificate to LPA by 07/24/2024.
Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure personnel record was maintained for two out of three staff which poses a potential health, safety, and personal rights risk to children in care.
Resolution: LPA discussed the requirement to maintain personnel records at the facility for all employees and volunteers. Licensee stated S3 will no longer assist/volunteer at the facility. Licensee agreed to provide copies of required personnel documents for S2 to LPA by 07/24/2024.
Based on observation, interview conducted, and records reviewed, the licensees did not comply with the section cited above as licensee did not ensure records were maintained for one child present which poses a potential health, safety, and personal rights risk to children in care.
Resolution: LPA discussed the requirement to ensure each child’s records are completed and maintained at the facility. Licensee agreed to provide proof of required documents for C4 to LPA by 07/24/2024.
Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure 15-minute sleeping logs were completed for one out of one infant which poses a potential health and safety risk to children in care.
Resolution: LPA discussed the requirement to check and log infant sleeping every 15 minutes for infants under 24 months. Licensee agreed to provide proof of completed 15-minute sleeping logs for C4 to LPA by 07/24/2024.
Based on observation, the licensee did not comply with the section cited above in that there was not a crib available for all infants in care, which poses an immediate health, safety or personal rights risk to persons in care. Licensee stated the two infants in care slept on bean bags but that they did have one pack n play which they would need to set up.
Resolution: Immediately, Licensee agrees to place infants to sleep in a crib and obtain enough cribs for the infants in care and submit proof to CCL by 7/10/23. Licensee also agrees to submit a written statement of their understanding of the infant safe sleep regulations.
Based on record review, the licensee did not comply with the section cited above in 2 out of 2 infants did not have 15 minuet sleep logs on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to start maintaining a 15 minuet sleep log for C3 and C4 and any other infants in care and submit proof to CCL by 7/14/23.
Based on observation, the licensee did not comply with the section cited above in that there was not a gate barricading the stairs during LPA inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they had a new gate which they will be installing to barricade the stairs. Licensee agrees to submit proof of the installed gate to CCL by 7/10/23.
Based on observation, the licensee did not comply with the section cited above in that scissors, vitamins, and cleaning supplies were accessible to children, which poses/posed a potential health, safety or personal rights risk to persons in care. When LPA arrived children were not in the areas where the hazardous items were observed.
Resolution: Licensee agrees to remove items and make them inaccessible. Licensee also agrees to place a gate blocking off the kitchen making it off limits and submit a proof to CCL by 7/10/23.
Based on record review and Licensees own admission, the licensee did not comply with the section cited above in that a disaster drill has not be conducted or documented, which poses/posed a potential health, safety or personal rights risk to persons in care. During inspection Licensee was unable to provide documentation of disaster drills.
Resolution: Licensee agrees to conduct a disaster drill and send documentation to CCL by 7/14/23.
Based on record review and Licensees own admission, the licensee did not comply with the section cited above in that a current roster was not on file, which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee was unable to provide a copy of their roster during the inspection.
Resolution: Licensee agrees to submit a current roster to CCL by 7/10/23.
Based on record review, the licensee did not comply with the section cited above in that 4 out of 4 children did not have LIC282 on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit copies of LIC282 for C1-C4 to CCL by 7/14/23.
