The UIR was received by the Riverside Child Care Regional office, on 04/29/24. The UIR documented an incident regarding supervision. Upon arrival, LPAs met with the Lead Teacher Lourdes Butman and stated the purpose of the visit. Records were reviewed and interviews were conducted. On 4/29/24 at 9:30am, while the children were transitioning from outside to inside the classroom, a child was left outside unattended. At 9:31am the child knocked on the door, and staff realized the child had been left outside. There was a total of 20 children and three staff present the date of the incident. UIR stated the Child's Parent/Guardian was immediately contacted about this incident. Along with the submitted UIR, Facility Administration included written statements from staff involved as well as a plan of action to correct and prevent this type of incident from occurring again in the future. The facility was found to be in violation of the following Title 22 regulation: 101229(a)(1) Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation.
