Based on observation, the licensee did not comply with the section cited above in that Licensee is currently on vacation for 5 days and left assistant(s) to run the daycare, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a signed and dated written statement acknowledging their understanding of this regulation along with their detailed plan in the event they exceed the daily 20 percent temporary absence. Licensee agrees to submit written statement to CCL by 7/18/25.
Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff present did not have a mandated reporter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit mandated reporter certificate (AB1207) for S1 and S2 to CCL by 8/11/25.

Resolution: Licensee agrees to submit immunization record (measles, pertussis, flu - written statement is needed if declining flu shot) for S1 and S2 to CCL by 8/11/25. TB is needed for S2.
