Based on record review, the licensee did not comply with the section cited above in that LIC9227 was on file for C1, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed copy of LIC9227 and submit a copy to CCL by 12/19/25. LPA provided a blank copy during todays inspection.

Based on record review, the licensee did not comply with the section cited above in that there was no 15 minute sleep log on file for C1, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to begin documenting their 15 checks on C1 while they are sleeping. Licensee agrees to create a log and submit a weeks worth of entries to CCL by 12/19/25. LPA went over and provided a copy of the infant safe sleep regulations in Spanish.

Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current CPR/First Aid certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete CPR/First Aid training (A,merican Heart Association, American Red Cross, or EMSA) and submit copy to CCL by 1/8/26.

Based on observations, interviews conducted, and records reviewed, the licensees did not comply with the section cited above as licensees did not ensure a roster was maintained at the facility which poses a potential safety risk to children in care.

Resolution: LPA reminded Licensees of the requirement to maintain a current roster at the facility. Licensees agreed to provide proof of completed roster to LPA by 01/03/2025.

Based on observations, interviews conducted, and records reviewed, the licensees did not comply with the section cited above as licensee did not ensure their mandated reporter training was completed which poses a potential safety risk to children in care.

Resolution: LPA discussed the mandated reporter training requirement and reminded Licensees to renew the course every two years. Licensees agreed to provide completed certificate for Licensee to LPA by 01/03/2025.

Based on record review and Licensees admission, the licensee did not comply with the section cited above in that both Licensees do not have current CPR certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Both Licensees agree to complete EMSA or ARC/AHA approved CPR and First Aid training and submit proof to CCL by 3/2/23. https://emsa.ca.gov
