Based on observation, the licensee did not comply with the section cited above in that a daily inspection log was not present during todays inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create a daily inspection log and submit a weeks worth of entries to CCL by 7/11/25.

Based on observation, the licensee did not comply with the section cited above in that a pool alarm was not present during todays inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain and install a pool alarm that meets ASTM F2208. Licensee agrees to send CCL a video of the working alarm by 8/4/25.

Based on observation, the licensee did not comply with the section cited above in that a body hook was not present during todays inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a body hook and attach it to their 12 foot rescue pool. Licensee agrees to submit a picture of the body hook attached to the 12 foot rescue pool to CCL by 8/4/25.

Based on observation, the licensee did not comply with the section cited above in that a life ring was not present during todays inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a life ring with a minimum exterior diameter of 17 inches and labeled as approved by the United States Coast Guard. The licensee shall have the safety equipment visible from the swimming pool and readily available for immediate use. Licensee agrees to submit a picture of the life ring to CCL by 8/4/25.

Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure personnel record was maintained for one out of two staff which poses a potential health, safety, and personal rights risk to children in care.

Resolution: LPA discussed the requirement to maintain personnel records at the facility for all employees and volunteers. Licensee agreed to provide copies of required personnel documents for S2 to LPA by 07/25/2024.

Based on observation, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure they renewed their EMSA pediatric CPR and first aid certification by the expiration date which poses a potential safety risk to children in care.

Resolution: LPA reminded Licensee to renew CPR and first aid certification every two years prior to expiration. Licensee agreed to provide proof of enrollment and/or completed certificate to LPA by 07/25/2024.

Based on observation, the licensee did not comply with the section cited above in that the home had dust and debris throughout the house, clutter throughout the house, grease stains on the walls and counters in the kitchen, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Immediately, Licensee agrees to clean home and submit pictures to CCL by 8/7/23.

Based on record review, the licensee did not comply with the section cited above in that Licensee was unable to provide mandated report training and does not know the date of the last training. which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that a current disaster drill was not on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that there was bleach on the kitchen counter, sticky pest traps in lower kitchen cabinets, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Immediately, Licensee agrees to remove hazardous items and/or make inaccessible and submit proof to CCL by 8/7/23.
