Based on record review, the licensee did not comply with the section cited above in 1 out of 2 staff did not have their mandated reporter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit mandated reporter (Child Care Provider) for S1 to Community Care Licensing by 3/13/26.

Based on record review, the licensee did not comply with the section cited above in 1 out of 2 staff did not have their immunization's on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit immunization's for S1 (TB, Pertussis, Measles, Flu - letter needed if declining flu shot) to Community Care Licensing by 4/2/26.

Based on record review, the licensee did not comply with the section cited above in that a daily inspection log of the drowning prevention safety features and safety equipment which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee created a log during inspection and documented todays inspection. Licensee agrees to continue to conduct daily inspections and agrees to submit a weeks worth of entries to Community Care Licensing by 3/6/26.

Based on observation, the licensee did not comply with the section cited above in that a rescue pole with a body hook was not available during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase a rescue pole with a body hook and a minimum fixed length of 12 feet and submit proof to CCL by 4/3/25. LPA went over and provided a copy of PIN 25-01-CCP.

Based on observation, the licensee did not comply with the section cited above in that a pool alarm was not present during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase and install an alarm that, when placed in a swimming pool, will sound upon detecting an entrance into the water, and submit proof to CCL by 4/3/25. LPA went over and provided a copy of PIN 25-01-CCP.

Based on observation, the licensee did not comply with the section cited above in that a pool life ring was not available during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase a life ring with a minimum exterior diameter of 17 inches and labeled as approved by the United States Coast Guard and submit proof to CCL by 4/3/25. LPA went over and provided a copy of PIN 25-01-CCP.

Based on record review, the licensee did not comply with the section cited above in 1 out of 2 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must submit proof of Mandated Reporter General and Child Care certificates for S2 to CCL by 01/06/2023. www.mandatedreporterca.com
