Based on interview observation, and record review, the staff does not have proof of immunization records during inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit the missing records to LPA by email by January 2, 2024.

Based on interview, record review, and observation, the licensee did not have the completed LIC9227 in three infants files during inspection. The licensee was not aware of the infant safe sleep regulations, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email the LIC9227 by 12/04/23 for current infant enrolled that is under 12 months.

Based on interview, record review, and observation, the licensee was not aware of the infant safe sleep regulation and the 15 minute check was not implemented, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee and staff will begin to implement the 15 minute checks during naps 11/28/23 and provide proof to LPA within 2 weeks by email.

Based on interview, observation, and record review, not aware of the safe sleep regulations and 15 - minute checks, three infants did not have a 15-min sleep chart filled out during inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will implement sleep charts for all infants 11/27/23 and provide proof to LPA after two weeks of implementation by email.

Based on interview, observation, and record review, the licensee and staff was not aware of the 15 minute checks and safe sleep regulations, three out of three infants enrolled did not have safe sleep documents, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee and staff will implement infant safe sleep regulations and start the 15 minute checks and documenting, looking for symptoms and documenting on a sleep chart. Ages 0-12 and Ages 12 - 24 months. The sleep chart will be completed and emailed to the LPA within two weeks of implementing.

Based on intervivew, record review, and observations, three staff need to complete the mandated reporter training, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide the mandated reporter certficates to LPA by 12/19/23 by email.
