Resolution: Licensee will email the LIC9227 by 12/04/23 for current infant enrolled that is under 12 months.

Resolution: Licensee and staff will implement sleep charts for all infants 11/27/23 and provide proof to LPA after two weeks of implementation by email.

Resolution: The licensee and staff will implement infant safe sleep regulations and start the 15 minute checks and documenting, looking for symptoms and documenting on a sleep chart. Ages 0-12 and Ages 12 - 24 months. The sleep chart will be completed and emailed to the LPA within two weeks of implementing.
