Based on observation interview & record review, the licensee did not comply with the section cited above in that Licensee familiy member Samantha Resse has been visiting for 1 month and does not have finger print clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have the finger prints completed by POC date.

Based on observation , the licensee did not comply with the section cited above in that Need plug covers in living room, outside playroom, kitchen, need safety gate near dog area, both entries to hall ways, both storage areas, remove laundry detergent or gate or barrier before the washer and dryers, safety locks on kitchen cabinets and drawers, lock on outside cabinet which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have corrections completed by poc date.

Based on record review, the licensee did not comply with the section cited above in that Licensee and staff missing mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Training will be completed by poc date

Based on record review, the licensee did not comply with the section cited above in Licensee and staff missing immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send proof of immunizations by POC date

Based on record review, the licensee did not comply with the section cited above in that All files missing LIC 282 Affidavit which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send proof of documentation via email.
