Based on observation and interview, the licensee did not comply with the section cited above. Upon LPA arriving at the facility, Licensee was not present at the home. LPA heard Child 3 crying from outside the facility.LPA called Licensee and confirmed she was not at home. Child 3 and Child 6 were in the home with 15 year old minor, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will provide written procedures on the protocol she will follow to avoid leaving a minor to supervise children. Licensee will submit written protocol to the Department by 07/29/2025.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee reported she documents the safe sleep daily and will shred it at the end of the day. Based on records reviewed, there were no entries for today's date. According to Licensee, Child 3 was asleep when she left the home,which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee was provided with a 15-minute Safe Sleep monthly template. Licensee will start documenting the sleep checks and will maintain forms in the child's file. Licensee will submit proof to the Department by 08/09/2025.

Based on observation and interview, the Licensee did not comply with the section cited above. Inside the accessible bathroom, LPA observed disinfecting spray bottle. In the accessible back yard, LPA observed two large dogs and observed multiple dog feces all over the mulch area and grass, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately made disinfecting spray bottle inaccessible. Licensee will remove all dog feces from the mulch and grass area. Licensee will submit pictures to CCL by 05/17/2024.

Based on observation and interview, the Licensee did not comply with the section cited above.LPA observed an above ground swimming pool. Licensee stated she installed it on August 2023. Licensee installed an above ground swimming pool without prior licensing approval. The pool is not fenced allowing windows and sliding glass door direct access. There is standing water in the bottom of the pool (about 1inch or less), which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will fence the above-ground pool in accordance with Title 22 regulations by 05/29/2024. A return visit will be made by LPA after 05/29/2024 to ensure the licensee has followed the above measures to clear this deficiency. Licensee stated she will make the backyard off limits until the above ground pool is fence in accordance with Title 22 regulations.

Three children’s files were missing LIC 995 A (Notification of Parents Rights and LIC 9150 (Notification of Additional Children in Care). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated will have parents complete all required documents and will provide pictures to CCL of completed forms by 05/17/2024.
