Based on observation, interview and record review, the licensee did not comply with the section cited above in that no 15 minutes sleeping check records was found at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee should comply with Safe Sleep Regulation for infant and provide a plan how to comply.
Based on observation and interview, the licensee did not comply with the section cited above in that LPA observed an infant sleeping in the playpen wearing a sleep sack during the visit on 6/25/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee should comply with Safe Sleep Regulation for infant and provide a plan how to comply.
Based on observation and interview, the licensee did not comply with the section cited above in that LPA observed an infant sleeping at the bedroom with the door closed during the visit on 6/25/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in that LPA observed and the helper stated that there were 5 infants and 1 preschool aged child in care, which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that LPA observed an infant sleeping in a playpen with a doll and a towel, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in that the Licensee was not at the facility during visit on 6/25/2025. Per the Helper, the Licensee had been out of the country from 6/23/2025 to 6/30/2025 noon, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee would provide a plan how to comply with the requirements of temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day.
Based on observation and interview, the licensee did not comply with the section cited above in that the helper's CPR and First Aid certificate was not found at the facility where the Licensee (via phone call) informed LPA that the helper would attend CPR and First Aid training on July 11&12, 2025, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee would arranged the helper to attend the CPR and First Aid training on July 11 &12, 2025.
At 2:45PM., Based on record review, interview and observations, LPAs confirmed licensee is operating over capacity with 5 infants in care. This poses an immediate health and safety risk to children in care.
Resolution: Licensee will lower her total enrollment to required 4 infant by the due date: 10/13/2023. Updated children shedule and children's roster will be submitted to the the department via email.
At 1:40PM., Based on observations, LPAs confirmed child, C1, awake in napping crib. This poses a potential health and safety risk to children in care.

At 3:15PM., Based on record review, interview, and observations, LPAs confirmed assistant present without proper criminal record clearances on file. This poses a potential health and safety risk to children in care.
Resolution: Licensee will ensure all staff received proper criminal record clearance and association prior to presence with children. Proof of correction will be submitted to the department via email.
At 2:55PM., Based on record review and interview, LPA confirmed licensee is not documenting infant napping conditions during each 15-minute review. This poses a potential health and safety risk to children in care.

At 1:45PM., Based on observations and record review, LPA confirmed licensee utilizing off-limit bedrooms, #1, #2, for napping services. This poses a potential health and safety risk to children in care.
Resolution: Licensee will submit an updated LIC999 to the department showing on/ off limit areas. Subsequent inspection will be done to add bedrooms to the day-care. Proof of correction will be submitted to the department via email
