Based on observation and record review, the licensee did not comply with the section cited above as she has not done safe sleep logs since May 2025. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA explained to licensee the regulation of maintaining safe sleep logs. Licensee agrees to conduct and document the safe sleep logs, daily, every 15 minutes for all children enrolled under 24 months. Licensee will submit proof of two week daily sleep logs for children enrolled under 24 months. Licensee will email proof to LPA Tapia-Mandujano by 7/11/25.
