Based on interview, the licensee did not comply with the section cited above as facility self reported that there was a child left alone outside during transition back to the classroom from the play yard, which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the facility did not comply with the section cited above as facility self reported that there was a child left alone inside the classroom during outdoor play, which poses a potential health, safety or personal rights risk to persons in care.

Based on results received from the certified water samplers, the facility exceeded 5.4 ppb of lead in their water source. This posed a potential health, safety, or personal rights risk to persons in care.

(b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s).
