Based on observation and interview, the licensee did not comply with the section cited above in three children were observed wearing sleep sack and there were blankets and pacifiers in the playpen/cribs, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will not use sleep sacks and will be sending sleep sacks home to parents. Licensee will not allow any objects in the playpen/crib while children are sleeping.
Based on observation, interview and record review, the licensee did not comply with the section cited above in 4 out of 4 infants were missing 15 minute sleep logs, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will maintain 15 minute sleep logs for all infants under 24 months old. Licensee will email LPA two week worth of sleep log for all infants by POC date.
Based on observations the licensee did not comply with the section cited above in two bedrooms doors were closed while children were sleeping, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands door to bedrooms must remain slightly opened while children are napping. Licensee need to be able to observe napping children without moving door. Licensee will be leaving the door open.
