Resolution: Licensees agrees to submitted Mandated Reporter Training certifications for all staff members to LPA via email by due date.

Based on interview and record review, the licensee did not comply with the section cited above: 1st Aid/CPR expired on 08/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submitt proof of registration or the CPR certification to LPA via email by dur date.

Based on observation, record review and statement from licensee, the licensee did not comply with the section cited above in 1 out of 1 fire drills and disaster drills were not complete, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire/ disaster drill and log the required informaation. Licensee will conduct drills every month therafter and send proof of completion to LPA by POC date.

Based on observation, record review and licensee statement, the licensee did not comply with the section cited above in 2 out 2 employees did not have proof of mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will complete Mandated Reporter Training and send proof of completion to LPA by POC date.

Based on observations and interview statement from licensee, licenseedid not comply with section cited above. Adult #1, did not have criminal record clearance while working in the facility, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have Adult #1, complete Live Scan and submit a copy of the receipt to LPA by POC date.

Based on record review, the licensee did not comply with the section cited above in 1 out of 1 Facility Roster was not complete which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete LIC 9040 Facility Roster and send completed roster to LPA by POC date. Licensee will keep roster readily available for Departments review.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 employees did not have a copy of the immunization on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain immunization for herself and staff and place a copy in personnel files. Licensee will send proof of immunizations to LPA by POC date.

Based on LPAs observation and interview statement, the licensee did not comply with the section cited above. Licensee was over capacity by 1 child which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee called the parent of a child and had child picked up right away. Licensee will read Title 22 Regulations 102416.5(a) regarding Staffing and Ration Capacity and write a sttement of her understanding of Ratio requirements.
