Based on observation, interview, the licensee did not comply with the section cited above, Licensee did not have a pool alarm nor an established log, which poses a potential health, safety or personal rights risk to persons in care
Resolution: Licensee agrees to meet the above noted regulation by acquiring a pool alarm and then establishing an log of its daily tests on or before the stated POC date of 09/15/2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee and assistant did not have current First Aid/CPR certificate available for review which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistant will complete/renew their First Aid/CPR Certrificate and provide proof of completion certificate to LPA on or before the stated POC date of 09/15/2025. Submissions can be made by text message, email, or US post sent directly to the Riverside Regional Office.

Resolution: Licensee agrees to meet the above noted regulation by acquiring the appropriate US Coast Guard Life Ring on or before the stated POC date of 09/15/2025.
Based on observation, interview, the licensee did not comply with the section cited above, Licensee did not have a 12 foot rescue pole present with body hook, which poses a potential health, safety or personal rights risk to persons in care
Resolution: Licensee agrees to meet the above noted regulation by acquiring the appropriate 12 foot rescue pole with body hook on or before the stated POC date of 09/15/2025.
Based on record review, the licensee did not comply with the section cited above which poses a potential health and safety risk to persons in care. During record review, LPA observed that C4 did not have a LIC 9227 on file.
Resolution: Licensee agrees to obtain a completed and signed LIC 9227 from the parent/guardian of C4 by 09/10/2024. Based on record review, the Licensee did not comply with the section cited above in which poses a potential safety risk to persons in care. During record review, LPA observed that the last fire/disaster drill was conducted/documented on 08/11/2024.
Resolution: Licensee agrees to conduct a fire/disaster drill with all children in care by 09/10/2024. Further, Licensee agrees to submit proof of completion of the fire/disaster drill to LPA via email. Licensee agrees to document the drill conducted and provide proof to LPA. Licensee agrees to conduct fire/disaster drills at least once every 6 months.
Based on observation, the licensee did not comply with the section cited above in which poses a potential health and safety risk to persons in care. During today's visit, LPA observed a infant sleeping in a playpen with a bib and sound device.
Resolution: Licensee agrees to submit a written statement of understanding to LPA acknowleding that she understands the Title 22 Regulations in regards to Infant Safe Sleep and loose articles in cribs or playpens by 09/10/2024. Based on record review, the licensee did not comply with the section cited above in which poses a potential health or safety risk to persons in care. During record review, LPA observed that LIC 995A is missing for C6. LIC 627 is missing for C3 and C4. Documentation of immunizations is missing for C6.
Resolution: Licensee agrees to obtain a completed and signed mentioned forms from the parent/guardian of C3, C4, and C6 by 09/10/2024. Based on LPA'S observation, interview and record review, the licensee Brenda Tucker, did not comply with the section cited above in at 2:35 PM, LPA observed the licensee and assistant caring for 5 children under the age of 2 years old, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee Brenda Tucker, stated they will ensure that will operate within the maximum allowed children, to include infants for ratio and capacity. The licensee will provide a written understanding of the Title 22 regulation for ratio and capacity and submit to community care licensing on or before the close of business day of 02/17/2023, by fax or e-mail.
Based on LPA interview and record review, the licensee Brenda Tucker, did not comply with the section cited above in the licensee completed CPR/1st Aid training through an unapproved vendor online, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee Brenda Tucker, agrees to provide proof of schedule to complete the preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code, through an EMSA approved vendor on or before 03/16/2023, by fax, mail or e-mail.
Based on LPA interview and record review, the licensee Brenda Tucker, did not comply with the section cited above in the licensee stated that she and her staff did not complete the mandated reporter training and was unaware of the requirement to complete the Mandated Reporter Training Part 1 & 2, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee Brenda Tucker, agrees to submit proof of Mandated Reporter Training Certificates for Part 1 & 2, the licensee and staff working with daycare children. Proof of training to the department on or before 03/16/2023 by mail, fax or e-mail.
