Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 staff memebers have expired CPR/1st Aid which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant have pre-existing appointment for CPR on April 11th and May 9th for 1st Aid. Licensee agrees to look for an appointment sooner and let LPA Danielson know if something has been scheduled. Licensee agrees to contact LPA Danielson if they are unable to find an appointment sooner to extend the POC date out.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 2 staff members, S2 has no TB or MMR on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain copies of immunizations and TB assessment for S2 by end of day on 02/21/2025 and send a copy to LPA Danielson at kayla.danielson@dds.ca.gov.

Based on record review, the licensee did not comply with the section cited above in 2 out of 6 children, C2 & C5 do not have copies of immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain copies of C2 and C5 immunizaitons records and send copies to LPA Danielson by end of day on 02/21/2025 at kayla.danielson@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in 1 out of 6, C4 is missing LIC995A from file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a signed LIC 995A, Parents Rights, from C4's parents and send a copy to LPA Danielson by end of day on 02/21/2025 at kayla.danielson@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in 2 counts of expired mandated reporters which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Assistant will completed mandated reporter training by 12/22/23 and submit certificates of completion to LPA Sims via email to sydney.sims@dss.ca.gov

Based on record review, the licensee did not comply with the section cited above in 1 count of Child's file missing completed LIC 700 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have LIC 700 filled out for child in care and maintain it in the child's file. Licensee will send copy of LIC 700 to LPA Sims via email to sydney.sims@dss.ca.gov
