Based on record review, the licensee did not comply with the section cited above in 3 count of Childen C3 - C5 not having immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee will obtain immunizations for children C3 - C5, or obtain a proper medical exemption by 1/20/25 and send copies of records to LPA Sims via email to sydney.sims@dss.ca.gov by 1/20/25

Based on record review, the licensee did not comply with the section cited above, Licensee and assistant do not have current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant will complete mandated reporter training by 1/20/25 and send copy of certificate to LPA Sims via email to sydney.sims@dss.ca.gov.

Based on record review the licensee did not comply with the section cited above in licensee did not have safe sleep log for infant under the age of 24 months.,which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review the licensee did not comply with the section cited above in 2 of 4 children's files reviewed did not have immunization record on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of immunization record for children to CCLD by 12/27/23

Based on record review one child in care under the age of 12 months did not have a infant safe sleep plan on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of LIC 9227 safe sleep plan for child to CCLD by 12/27/23
