Based on record review, the licensee did not comply with the section cited above in one child (C1) was missing LIC 995 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get LIC 627 filled out by authorized representative by 12/1/25. Send copy of document to LPA at tammy.dutra@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above in one out of one fire extinguisher was not charged which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee ordered a 2A10BC fire extinguisher during the inspection. The licensee stated she will send a picture of the fire extinguisher when she has it installed.

Based on observation, interview and record review, the licensee did not comply with the section cited above in two out of two employees files did not contain proof of current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will complete mandated reporter training and have her assistant complete the training by 7/1/24. The licensee stated she will send LPA copies of completion.

Based on record review, the licensee did not comply with the section cited above in one out of three staff files did not contain tuberculosis test results which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will submit test results for one staff to LPA Cunningham at nicolette.cunningham@dss.ca.gov.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as no sleep checks were documented which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated moving forward all sleep checks will be documented. The licensee will send proof of documented sleep checks to LPA Cunningham at nicolette.cunningham@dss.ca.gov.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in all children's files did not contain consent for medical treatment which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will have all parents sign consent for medical treatment and send proof to LPA Cunningham at nicolette.cunningham@dss.ca.gov.

Based on observation, interview and record review, the licensee did not comply with the section cited above in two out of three staff files do not contain immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will obtain immunization records and send to LPA Cunningham at nicolette.cunningham@dss.ca.gov.
