Based on record review, the licensee did not comply with the section cited above, Licensee does not have current Mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will completed required mandated reporter training by 1/30/24 and send copy of certificate to LPA Sims via email by 1/30/24 to sydney.sims@dss.ca.gov

Based on observation and record review, the licensee did not comply with the section cited above, Licensee and 2 assistants providing care for 27 children with a Licensed capacity of 14 which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will review regulations provided and write statement agreeing to follow the regulations and provide a plan of how the Licensee intends to stay in capacity compliance. Licensee will have guardians of all children enrolled sign LIC 9224 by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this. Licensee will send copies of LIC 9224 and statement to LPA Sims to sydney.sims@dss.ca.gov.

Resolution: Licensee will update Child Care Roster to have all enrolled or previosuly enrolled children. Licensee will send copy of updated Roster to LPA Sims via email to sydney.sims@dss.ca.gov by 8/22/24
